Research Literature Review Breast Cancer in USA Essay Introduction Breast cancer continues to be an important focus of attention for medical professionals, policy-makers and general public in the US. The high incidence of the disease and its devastating, in many cases, fatal, outcomes rivet the attention of the professional public. The researchers arrive at the conclusion that, given the availability of subsidized mammograms for poor Latina women, the main deterrent for improvement in the screening rate is lack of knowledge about cancer risks and treatment options. Although the women had some prior knowledge of cancer (ââ¬Å"the majority of baseline study participants (over 80%) knew age and family history as risk factorsâ⬠), the post-exposure assessment revealed a significant increase in knowledge after the program. Krystal Ngoc-Thy Luong and Jenny K. Yi in ââ¬Å"Apartment-Based Breast Cancer Education Program for Low Income Vietnamese American Womenâ⬠evaluate a similar program targeting a different ethnic group. As in the case with Latina women, breast cancer is the leading cause of death in this subgroup. Although Asian/Pacific Islander American, or APIA, women have a lower incidence rate of breast cancer (102.0 per 100,000 versus 140.8 per 100,000 for White American women), exposure to Western lifestyle tends to push these rates up. In addition, APIA women tend to be younger when diagnosed with breast cancer than other categories. The study aimed ââ¬Å"to develop, implement, and evaluate the effect of an apartment-based breast cancer educational program for the hard to reach low income Vietnamese womenâ⬠(Luong, Yi, 2005). The program was designed to be based on apartment visits as it provided a convenient way to reach women, covering neighbourhoods with high concentration of Vietnamese families in Houston. The intervention was carried out by two educators who delivered 20 educational sessions over the period of 3 months that provided the Vietnamese women with information concerning screening, recommendations and referrals to screening. Women were supplied with promotional materials including printed educational information and videos. Measurement of the knowledge levels through telephone surveys concerning breast cancer issues demonstrated that they were ââ¬Å"significantly higher in the intervention group after receiving education about breast cancer when compared to women in the control groupâ⬠(Luong, Yi, 2005). The researchers drew the conclusion that an apartment-based program has many advantages as compared to other ones because it addresses women in comfortable home settings, giving them a chance to relax and open up more to the conversation with the educator. Environmental Hazards and Breast Cancer Another important direction of research concerns the impact of pollutants and other environmental factors on incidence of breast cancer in women. The study covers ââ¬Å"chemicals that may be breast carcinogens, promote growth of breast cells and hormonally sensitive tumors, or affect mammary gland development and susceptibilityâ⬠(Brody, Rudel, 2003). The researchers identify already known breast cancer risk factors including reproductive factors, exposure to pharmaceutical hormones, diet, ionizing radiation, socioeconomic status and posited the need to identify more factors. Thus, the scholars point to the need to further investigate the risk associated with occupational hazards including exposure ââ¬Å"to the mammary carcinogens benzene, PAHs, and certain organic solventsâ⬠(Brody, Rudel, 2003). For instance, speaking of organic solvents, one study identified a connection between breast cancer rates in Danish women and their exposure to these substances in the workplace. Other studies demonstrated increased risk of breast cancer development for women employed in the chemical industry, textile manufacturing, or nuclear power plants. The authors conclude by saying that more research is needed in the area, but the literature available so far clearly indicates that the link exists between cancer rates and environmental factors. Treatment Options Finally, another issue which has been identified to inspire researchers is the availability of treatment options for cancer and choice between various alternatives. Kathleen B. Donaghy in ââ¬Å"Fostering Sound Medical Treatment Decision Making: A Focusing on Treatment Choices for Breast Cancerâ⬠explores this problem. Invoking the decision-making theory, the author evaluates the potential for adequate decision-making in patients struck with this shocking diagnosis. Donaghy (2003) believes that hasty decisions concerning treatment options made on insufficient information are often regretted by the patient later on. As common deterrents that prevent responsible decision-making she identifies five key points: 1.) cognitive overload 2.) failure to fully comprehend treatment options 3.) inability or refusal to process information in the presence of a denial defense 4.) hurried presentation of information by the physician 5.) self-imposed time constraints to get decisions made. (Donaghy, 2003) The article identifies the key strategies that can be employed by the physician to help the patient in the choice, such as providing maximal possible information to guide the patientââ¬â¢s decisions, improving communication, framing information in an appropriate format, obtaining informed consent etc. Using the decision theory to define decision-making as a choice between status quo and other alternatives shed light on patient values that can be involved in decisions regarding breast surgery. Conclusion The pervasiveness of breast cancer in the US makes research related to the issue highly topical. Examination of causes including environmental factors can help craft governmental policies that will prevent the spread of the deadly disease. On the other hand, developing policies aimed at early detection and effective treatment can be instrumental in curbing the death rate from breast cancer. In this light, creation of programs specifically tailored to the needs of individual groups can be an effective solution. Finally, research oriented toward helping the patient make the best decision concerning treatment options will be an important prerequisite of effective treatment programs. Experimental Investigations of Catalytic Effect of Cu2+ Experimental Investigations of Catalytic Effect of Cu2+ During Anodic Disolution of Iron in NaCl Electrolyte R.K Upadhyay1, Arbind Kumar2 and P.K Srivastava3 Abstract: Taguchiââ¬â¢s orthogonal array L9 has been effectively used to study the effect of process parameters such as voltage, feed rate and electrolyte concentration on material removal rate in context of two different types of electrolyte namely aqueous NaCl solution and electrolyte solution containing Cu2+ ions. The results indicated that Cu2+ has a catalytic effect on the anodic dissolution of iron, which restrict the oxidation of Fe2+ to Fe3+ and increases the dissolution rate during machining. The experimental results were analyzed using analysis of variance (ANOVA) method to investigate the significance and percentage contribution of individual process parameters on performance characteristics. Key Words: Electrochemical Machining, Aqueous NaCl, Cu2+, Parameters, Oxidation, Material Removal rate. Introduction: Electrochemical machining (ECM) has got an industrial importance due to its capability of controlled atomic level metal removal1. It is an anodic dissolutionà process based on electrolysis, where the application of a more traditional process is not convenient. ECM has been successfully employed in aerospace, automobile industries and now gaining much importance in the electronics and other high-tech industries for the fabrication of micro components2-3. Mask less and through mask electrochemical micromachining techniques have been successively used thin films and foils of materials those are difficult to machine by other methods4-5. Electrochemical machining is low voltage (5-25 volt) machining process which offers high metal removal rate and also capable to machine hard conductive materials into complicated profiles without any thermal damages thus suitable for mass production work with low labor requirements6-7. The dissolution rate is highly reliant on the selection of electroly tes and its current carrying capacity. On increasing the concentration of electrolyte solution dissolution rate also increases but excess concentration allows the crystal formation, which may damage the accessories of ECM and reduce the volume of electrolyte in flow pipes. The conductivity of electrolyte depends not only on the concentration but also on ionic interaction. Thus, the current carrying process done by the base electrolyte is small, but H+ and OH ions produced in electrolysis of water play important role8-9. The achievement of higher dissolution rate in ECM is a strong research base which is possible by change in composition of electrolyte solution to promote catalytic effect during dissolution10. During electrochemical machining of iron at low current density it has been observed that Fe+ cation formed very easily but it is highly unstable and immediately oxidizes into Fe2+ state. Increase in current density leads to simultaneous production of Fe2+ and Fe3+, at higher current density apparent valence of iron increases above three11. Therefore, to stabilized Fe2+ in the aqueous solution is a challenge during dissolution. EXPERIMENTAL SET-UP AND PRINCIPLE OF ECM: Fig 1 Experimental set-up ECM is an anodic dissolution process works on the principle of Faradays law. While machining of iron in presence of aqueous NaCl electrolyte solution the following chemical reactions are observed12. Reactions at Cathode: Na+ + e à ¯Ãâà ³ Na Na + H2O à ¯Ãâà ³ NaOH + H+ 2H+ + 2e à ¯Ãâà ³ H2 It shows that only hydrogen gas will evolve at cathode. When pure iron is being machined electrochemically the following reactions would occur13-14. Fe à ¯Ãâà ³ Fe2++ 2e Fe2+ + 2Cl à ¯Ãâà ³ FeCl2 Fe2+ + 2(OH) à ¯Ãâà ³ Fe(OH)2 FeCl2 + 2(OH) à ¯Ãâà ³ Fe(OH)2 + 2Cl 2Cl Cl2 + 2e 2FeCl2 + Cl2 à ¯Ãâà ³ 2FeCl3 H+ + Cl à ¯Ãâà ³ HCl 2Fe(OH)2 + H2O +O2 à ¯Ãâà ³ 2Fe(OH)3 Fe(OH)3 + 3HCl à ¯Ãâà ³ FeCl3+ 3H2O FeCl3+ 3NaOH à ¯Ãâà ³ Fe(OH)3 + 3NaCl It shows that during electrochemical machining of iron in NaCl electrolyte, iron is removed as Fe(OH)2 and precipitated as sludge while sodium chloride is recovered back. Due to further reaction, formation of Fe(OH)3 is also possible Which, confirms the existence of iron in +2 and +3 states during dissolution. Determination of Fe2+ and Fe3+ ions in electrolyte solution: The electrolyte solution containing Fe+2 and Fe+3 ions was collected. Fe+2 ions were determined directly by titrating a known volume of iron electrolyte solution with K2Cr2 O7 in acidic medium (HCl). Cr2O7 2- + 6Fe+2 + 14H+ = 2Cr+3 + 6Fe +3 + 7H2O Internal indicator N- phenyl anthranilic acid was used to mark the end point. Fe+3 ions were determined after all the Fe+3 ions are reduced into Fe+2 ions with SnCl2 in presence HCl in hot. Sn+2 + 2Fe +3 = Sn+4 + 2Fe+2 The solution was then cooled and excess SnCl2 was removed by adding HgCl2 solution. 2Hg+2 + Sn+2 +Cl = Sn+4 +Hg2Cl2 (white ppt) Titration of known volume of standard solution was done using standard solution of K2Cr2O7 in acidic medium. From the volume of K2Cr2O7 used, the total amount of Fe+2 and Fe+3 ions was determined. The amount of Fe+3 ion was determined by subtracting amount of Fe+2 which is determined earlier. Material removal rate during electrochemical machining is greatly influenced by dissolution valence. As the dissolution valence decreases MRR increases. In this paper an approach is made to enhance the electrochemical dissolution of iron through control of valency (transition) therefore, in this direction, use of electrolyte solution containing Cu2+ is suggested. The dissolution limit of iron by Cu2+ ions can be is justified by considering the standard electron potential Eà ° for Cu2+, Fe/Fe2+and Fe/Fe3+ described as follows15. Cu2+ + 2e- Cu Eà ° = +0.34V Fe2+ + 2e- Fe Eà ° = -0.44V Fe3+ + e- Fe2+ Eà ° = +0.77V As Eà ° for Cu2+ Cu is more positive than Fe2+ Fe, Cu2 +will oxidize Fe to Fe2+. However, as Eà ° for Cu2+ Cu is less positive than Fe3+ Fe2+, Cu2+ will not oxidize Fe2+ to Fe3+. Making electrolyte solution: 250 gramsof NaCl was mixed with400 gramsof CuSO4 in10 litersof water. The mixture is stirred well for 2 minutes then heated until it loses its green color. The crystals of sodium sulphate (Na2SO4) and copper chloride (CuCl2) were removed by filtering the solution and thi the solution thus obtained was saturated solution of Na2SO4 containing Cu2+ ions which participates in anodic dissolution process. MACHINING CONDITIONS: Following machining parameters are selected on the basis of performance characteristics, Table1: Machining conditions for analysis SELECTION OF MACHINING PROCESS PARAMETERS Table 2 shows machining parameters and selected levels for experimental procedure Table 2: Process parameter and their levels Measurement of MRR The initial weight of the work piece was taken for calculation of MRR. Keeping the flow rate constant at 15 lit/min and the rest of the parameters are set according to table 1 for each run. Work piece was kept horizontal, and cylindrical electrode was used for machining. Gap between tool and workpiece was maintained carefully to avoid the choking. The electrode was fed continuously towards the work piece during machining and time was recorded. After machining, the cavity was formed on the work-piece. (1) EXPERIMENTAL PROCEDURE: The design resulted in total of eighteen experiments, which are performed at 10V-18V supply voltage, 10-30 g/lit electrolyte concentration and 0.0001-0.0005 cm/sec feed rate as the values for the control variables. The responses measured are Material removal rate (MRR) Scheme of the experiments is as shown in Table 3. The graphs shown in figure 3 are plotted from the regression equation (2). Fig 3. As seen from the plot obtained, the MRR increased with increase in both voltage and feed rate. This is due to the fact that with increase in voltage the current increases in the inter electrode gap thus increasing the MRR. Feed rate is another important parameter. Increase in feed rate results in decrease of the conducting path between the workpiece and the tool hence resulting in high current density thus enhancing the rapid anodic dissolution. An overall increase in the MRR was also observed with increase in the concentration as the larger number of ions associated with the machining process which increases the machining current and thus results in higher MRR. Effects of selected process variables (i.e. Voltage, Feed rate and Concentration) on material removal rate (MRR) at different sets of conditions while machining in presence of aqueous NaCl solution are shown in figure 4(a), 4(b) and 4(c). Fig.4(c) Effects of Concentration on material removal for different Feed rates, Voltage= 14 V NaCl electrolyte tend to promote the oxidation of Fe2+ to Fe3+ during the dissolution process the maximum MRR obtained during machining of iron in aqueous NaCl solution recorded was 0.0653 cm3/sec. Although the higher concentration of NaCl is favorable for better MRR but excess concentration allows the crystal formation which reduces the volume of electrolyte in flow pipes and also affects the dissolution rate. Analysis of variance when machining in presence of electrolyte solution containing Cu2+ ions Percentage contribution of each parameter on material removal rate during electrochemical machining of iron in electrolyte solution containing Cu2+ ions is shown in table 5 and represented graphically in figure 5. Main Effects Plot for SN ratios (electrolyte solution containing Cu2+ ions) The oxidation of Fe2+ in to Fe3+ is restricted due to the presence of Cu2+ in electrolyte solution which promotes the higher dissolution rate during machining. The maximum MRR obtained during machining of iron in presence of Cu2 electrolyte solution containing Cu2+ ions was 0.0774 cm3/sec, which is 18.5% more when compared with aqueous NaCl electrolyte. CONCLUSION The electrochemical characteristics of iron in aqueous NaCl solution and electrolyte solution containing Cu2+ ions has been analyzed experimentally to investigate the influence of process parameters on MRR. The Process parameters such as voltage, feed rate, Electrolyte concentration, were successfully controlled. The different combinations of these parameters were used for the experimentation in order to determine their influence on MRR. The experiment was performed by varying all parameters in combination as per L9 orthogonal array. The experimental observations support the conclusion that the presence of Cu2+ ions in electrolyte solution restrict the further oxidation of Fe2+ to Fe3+ and enhance the low valence dissolution of iron during machining. Design of experiments and analysis of variance helped in identifying the significant parameters affecting MRR. The best combination of the parameters are Voltage= 18 V, Feed Rate=0.0005 cm/sec and electrolyte Concentration = 20 g/lit whe n using a solution containing Cu2+ ions as electrolyte. The maximum MRR obtained was 18.5 % higher when compared with aqueous NaCl electrolyte for the same set of working conditions. The Power of Internet Feedback Eight Sources I. Introduction Feedback is one of the most important elements to a businesses success. No matter what the business, all have to deal with two constants, customers and customer satisfaction. These two things lay the groundwork for a successful business. The customers come from various types of media strategies such as advertising, mail-order publications, public relations, retailing and merchandising, sales, market research, and prices of goods. In order for a business to be competitive in its market the business must know what the customer wants, what price the customer is willing to pay, why the customer would buy one brand over another and other feelings that customers need met that would make them buy your product or service. Great customer service is the difference that makes the difference, whether your customers deal repeatedly with one representative of your company or with a different person each time they call, whether you do business face-to-face or electronically. Internet feedback is the new wave as far as getting feedback from customers, "A company can't create advocates without a true understanding of customer preferences. Not surprisingly, much of the innovation and groundbreaking techniques for "listening" to customers is coming from the Web world (Griffin.)" This is why Internet feedback is one of the most useful and resourceful tools a business or institution can have at its fingertips today. II. Literature Review and Findings The literature I found to be most relevant to the type of information that I deemed most important covered a websites ability to satisfy a customer having specific feedback goals from the website. In an essay written by Jakob Nielsen and Marie Tahir called, Design Usability: Keep Your Users In Mind Incorporating Feedback at Several Stages into an Ongoing Design Process Will Help a Site to Thrive. In this article the authors describe how the Web, like nothing else, has taught people how easy it is to walk away from a poorly designed product. The truth with the person who could easily and swiftly go to your website they can leave just as fast. A person who has invested the time to drive to a mall might be willing to suffer through a long checkout process much more readily than a Web customer, who can just as easily click over to the competition.

Euthanasia: Killing or Helping Is society playing the role of God or is the world so wrapped up in their lives that God no longer matters? Euthanasia has been around since the ancient Romans and Greeks and has been a highly debated subject just as it is today. In history and in arguments stated today is that ââ¬Å"people are the created and not the Creatorâ⬠(Gula 26). There are many things that society can argue about the subject of euthanasia but the main debate is that euthanasia and physician-assisted suicide is wrong. Society gets euthanasia and physician-assisted suicide confused because they both have to do with physicians tending to the patientââ¬â¢s death. Society is either for or against euthanasia and physician-assisted suicide. It is debated throughout history, within the church, and even within the medical profession; however euthanasia is wrong. Euthanasia is a problem that has been facing people since the time of the Ancient Greeks and Romans. Manning states that the ancient Greeks and Romans preferred to die, then to go through the pain and suffering (Manning 6). During the time of the Romans and Greeks, euthanasia and physician-assisted suicide was a common, everyday thing. However, a group called the Pythagoreans opposed euthanasia back then because they believed that God valued the human life itself and the act of killing someone who is already suffering was considered disrespectful (Manning 6). Plato and Aristotle both opposed euthanasia for a couple of reasons. Plato opposed suicide, but on the other hand, he rejected that the right to take a life had only belonged to the gods (Manning 8-9). According to Manning, Plato believed when a personââ¬â¢s life was considered useless... ...at kind of relationship would that constitute for a physician? Euthanasia is a subject that society is not just going to let pass by them without society giving their points of view. Euthanasia is just another excuse for physicianââ¬â¢s to be able to kill another human life that could still be worth living, just as abortion is seen. As society has grown since ancient times they have come across many more debates and many more reasons why euthanasia so be allowed or not allowed. If society allows such an act of degrading of a life, we will be taking life into our hands and will be leading into a whole different world. The main question that boggles many peopleââ¬â¢s minds is that if society allows this form of killing to go on then what is society going to let happen next? Is society taking life into our own hands or is our life taking us into its hands?

Internal fetal monitoring is accomplished with a fetal scalp electrode that is a direct electrocardiogram of the FHR and therefore produces the most accurate FHR tracing having an advantage over the external monitoring. The FSE is attached to the fetus during a vaginal exam and then connected to a fetal monitor. Because the risk of transmission to the fetus is increased by the small puncture in the fetal scalp, use of internal scalp electrodes should be avoided if at all possible in the presence of known maternal infections such as HIV, hepatitis or GBS. Fetal scalp monitors are also avoided in preterm infants because of the increased risk of ventricular hemorrhage. Electronic monitoring of UCs can be done internally by using an intrauterine pressure catheter (IUPC). It is inserted into the uterine cavity through the cervical os. It reflects the pressure inside the uterine cavity. As the pressure changes, it traces on the graph paper. The IUPC can measure the resting tone of the uterus between contractions, referred to as intensity. An advantage of an IUPC is that it provides a near-exact pressure measurement for contraction intensity and uterine resting tone. The sensitivity of the IUPC allows for very accurate timing of UCs, thus making it extremely useful when closer uterine monitoring is needed. A disadvantage for both internal monitoring methods is that membranes must be ruptured and adequate cervical dilation must be achieved for insertion. The procedure is invasive and increases the risk of uterine infection or perforation or trauma. It can also cause a placenta rupture if the placenta is low-lying. Electronic FHR monitoring can be done externally by using an ultrasound (US) transducer. The transducer is placed on the maternal abdomen over the fetal back and held by an elastic belt. The US transducer can be more beneficial than auscultating the FHR because it provides a continuous graphic recording. It can show the baseline variability and changes in the FHR. It is noninvasive and doesnââ¬â¢t require the rupture of membranes or minimal cervical dilation. FHR monitoring by US transducer is limited because it is susceptible to interference from maternal or fetal movement and may produce a weak signal. The tracing may become sketchy and difficult to interpret. Telemetry is another type external monitoring. It can monitor both FHR and uterine activity. This system can be worn on a shoulder strap by the mother, which allows the woman to ambulate, helping her ambulate while continuously monitoring. Electric monitoring of UCs can be done externally using a tocodynamometer or tocotransducer (toco). The toco is placed on the maternal abdomen at or near the fundus and held in place by an elastic belt. As the uterus contracts, pressure exerted against the toco is transmitted and recorded on to graph paper. The toco can assess UCs for frequency and duration, but not intensity. The advantages are that it is non-invasive, easy to place, and may be used both before and following rupture of membranes. It also provides a permanent, continuous recording of the duration and frequency of contractions. Explain what Pitocin is used for: Pitocin is used for the induction and/or augmentation of labor at term, facilitation of threatened abortion, and in postpartum to control bleeding and prevent hemorrhage and uterine atony after expulsion of the placenta. PELVIC ASSESSMENT FINDINGS:Give normal findings and measurements Diagonal conjugate ââ¬â extends from the suprapubic angle to the middle of the sacral promontory. ââ¬â 12.5 cm Ischial Spines ââ¬â arise near the junction of the ileum and ischium and jut into the pelvic cavity They serve as a reference point during labor to elevate the descent of the fetal head into the birth canal. 10.5 cm Pubic Arch ââ¬â triangular space below the symphysis pubis. The head passes under this arch during birth. 1.5 ââ¬â 2 c from diagonal conjugal Coccyx -small triangular bone that articulates with the sacrum. It usually moves backward during labor to provide more room for the fetus. > 8cm in diameter Sacrum ââ¬â wedge-shaped bone formed by the fusion of five vertebrae. On the anterior upper portion of the sacrum is the sacral promontory which is another guide in determining pelvic measurements. Types of Pelvis Gynecoid -The most common female pelvis is the gynecoid type. The inlet is rounded, with the anteroposterior diameter a little shorter than the transverse diameter. This is the most favorable for a vaginal delivery. Android ââ¬â The normal male pelvis is the android type; however, it occasionally is seen in females. The inlet is heart-shaped. Anthropoid ââ¬â The inlet of an anthropoid pelvis is oval, with a long anteroposterior diameter and an adequate but rather short transverse diameter. This is the second most favorable for vaginal delivery.

Poem Red to You vs Reading a Poem Essay
I believe that having a poem read to me is more powerful then reading a poem off of a page. When I close my eyes I can see the words that the reader is speaking, instead of trying to understand what the author is attempting to write to me; so it is easier for me to understand what the author is communicating without decoding the words. When I close my eyes as someone is reading, the reader paints a picture in my head of what the author has written and I can also see the words that are being spoken. The poem being read out loud helps me to get a better understanding of what the author was trying to say without reading between the lines. Having the poem read to me also helps me find different authors that I might not have looked at before or even heard of . So it gives me a bigger variety of the authors I might enjoy reading and an opportunity to ask any questions about what is being read. When I read a poem straight off a page I donââ¬â¢t not know what words to say strongly, it is like Iââ¬â¢m reading a really boring article in the news paper or reading a story with no expression. For my logic to work I must assume the person reading to me knows the poem and knows the words to emphasize. Therefor having a poem read to me is more powerful then reading a poem off of a page because I get a better understanding of what is said, I can ask questions. Especially if the reader uses the proper emphasis on key words.
